Instructions in English:

1. Filling up of this form does not guarantee admission. Students must visit the Academy Office for final registration.
2. Students are not required to submit fees at the time of office registration. They may avail up to 7 days of demo classes to assess the academy, faculty, and curriculum before submitting fees as per academy instructions.
3. Documents required for registration at the Academy Office:
   (i) Passport size photographs – Qty: 02
   (ii) Copy of Aadhar Card
   (iii) Copy of 10th or 12th Marksheet (or both, whichever is applicable)
   Original documents must be produced for verification.
4. In case any wrong, misleading, or false information or documents are submitted, the admission may be cancelled at any time.

हिंदी में निर्देश:

1. इस फॉर्म को भरने से प्रवेश की गारंटी नहीं मिलती है। अंतिम पंजीकरण के लिए अकेडमी कार्यालय में आना अनिवार्य है।
2. कार्यालय में पंजीकरण के समय किसी भी प्रकार की फीस जमा करना आवश्यक नहीं है। विद्यार्थी अधिकतम 7 दिनों की डेमो क्लास ले सकते हैं।
3. कार्यालय में पंजीकरण हेतु आवश्यक दस्तावेज़:
   (i) पासपोर्ट साइज फोटो – 02 नग
   (ii) आधार कार्ड की प्रति
   (iii) 10वीं या 12वीं की मार्कशीट की प्रति (या दोनों, जो लागू हो)
   सत्यापन हेतु मूल दस्तावेज़ प्रस्तुत करना अनिवार्य है।
4. यदि किसी भी प्रकार की गलत अथवा फर्जी जानकारी या दस्तावेज़ प्रस्तुत किए जाते हैं, तो किसी भी समय प्रवेश रद्द किया जा सकता है। I have read and agree to all the above guidelines.
